We are so excited to have your child at VBS!

Other than listed parents/legal guardians, who else has permission to pick up your child from VBS? Individuals picking up children from VBS MUST show photo ID

Does your child have any allergies/dietary restrictions or medical concerns? Anything else you want us to know about your child? Note: we can not make all dietary substitutions, but will omit snack ingredients to avoid allergies

I hereby release Jerome Christian Church and its staff and volunteers from responsibility and liability for any injury or illness that my child may sustain during VBS activities. In the event of an emergency where I cannot be contacted or present, I consent to any reasonable medical treatment as deemed necessary by a licensed physician, surgeon or dentist. In the event that my child requires treatment by a physician and/or hospital personnel, I agree to hold such person free and harmless of any claims, demands, suits or damages arising from giving of such consent. I also acknowledge that I will ultimately be responsible for the cost of any medical care not covered by insurance. By entering your name here, you agree to these terms.

I give permission for my child's photo to be used on Jerome Christian Church's website and/or social media pages. Example would include highlights of the day and week of VBS.
